Please read all instructions before you complete your report:
Purpose
The Lompoc Police Department has created a Citizen Crime Report form that allows citizens to file certain reports without the personal service of an officer. This system will allow sworn officers of our department to spend additional time investigating more serious offenses and will make them available to respond to emergency calls for service. These types of reports are available at the front counter, they may be downloaded via the internet and faxed to the Police Department, or they may be mailed to a citizen for completion. Completed reports may be returned to the police department in person, via fax, or by mail.
What Types of Crimes/Incidents
This Citizen's Crime/Incident Report Form can be used for any of the following:
- Theft of Property value under $950
- Lost Property (cell phones, wallet, purse, etc)
- Vandalism
- Identity Theft
- Theft of Social Security #'s
- Theft of bicycles
Do Not Use this Report Form If:
- A suspect was observed.
- The victim is reporting a crime that is in progress or has just occurred.
- Weapons or violence are involved.
- Forced entry of a residence or vehicle was accomplished.
- Any physical evidence of the crime is present.
How to Make a Report
- You may only report incidents that occurred within the incorporated city limits of the City of Lompoc. If the offense / incident you are reporting occurred in another jurisdiction, please contact the appropriate Police or Sheriff's Department.
- Use this form when the crime was not initially reported and is now being made at a later date for insurance purposes only.
- Use one report form per incident. You may make additional copies of the offense / incident report form to report other incidents.
- Additional narrative sheets, property description sheets, and / or victim sheets may be attached as necessary.
- PLEASE PRINT LEGIBLY IN INK USING BLOCK LETTERS.
- Date and sign the report form prior to submission.
- Once the form is completed, return it to the Police Department in person, by mail, or by fax.
- Return this form within ten (10) days.
- Completed forms may be mailed, faxed or returned in person to:

What Happens Next?
- A case number will be assigned upon receipt and your report will be entered into the records management system.
- Please allow ten (10) working days for your report to be processed.
